This form can be helpful if you are a parent that would like to ask the court to start a parenting time arrangement for your minor child when you and the child’s other parent cannot reach an agreement on what the arrangement should be. You should only complete this form packet if you and the other parent were never married.

What are the qualifications?

  1. You have never been married to your child’s parent, and
  2. The court has issued a paternity order, or you and the other parent signed a paternity affidavit, and
  3. The court has not issued a parenting time order, and
  4. You would like to start getting parenting time.
  5. You and the other parent do not agree if or how you should start getting parenting time.
