Settlement Conference Request Form

If your mortgage lender has filed a foreclosure on your home, you may be entitled to a court ordered settlement conference with your lender to negotiate an agreement that could allow you to avoid foreclosure.

What are the qualifications?

  1. This is for homeowners who pay a mortgage.
  2. Your mortgage lender must have filed to foreclose on your home.
  3. There is no financial cost to you for participating in this settlement conference.
  4. You must notify the Court that you desire a settlement conference within 30 days of receiving the Notice by doing any of the following:
    • Fill out the Settlement Conference Request Form and return it to the Court at the address shown on the Summons, then send a copy of the form to the lender’s attorney at the address shown on the Summons, OR
    • Call the Court and request a settlement conference at the phone number shown on the Summons, OR
    • Go to the Court and request a settlement conference.

Wait for Court to Set Conference Date

After filing your forms, the court should schedule a settlement conference to take place within 25 to 60 days.
