License Suspension

These forms may be helpful if you have a suspended driver’s license. There are two forms included that you can complete: the Specialized Driving Privileges forms, and the Reinstatement Fee Waiver. The Specialized Driving Privileges forms ask the court for permission for you to drive to specific locations and the Reinstatement Fee Waiver forms asks the court to waive the fee to reinstate your driver’s license. Both can be completed and filed if needed. Form Helper option is available.

Before Starting

We recommend you set aside 1-2 hours to complete all forms. Representing yourself in court should not be taken lightly, and there are many reason why hiring an attorney is a good idea.   1. A certified copy of your OFFICIAL DRIVING RECORD. It can be purchased here: Official Driving Record. You can learn more about how to read your driving record here: Reading Driving Record.
  2. Insurance Verification, which you can get here: Insurance Verification.
  3. A copy of your driver’s license and your driver’s license number.
  4. A full list of the locations and addresses you would like to be able to drive to.
  5. A clear reason for needing specialized driving privileges.

For the reinstatement fees waiver forms, you will need the following:

  1. A copy of your driver’s license and your driver’s license number.
  2. A full list of your current expenses.
  3. A clear reason for needing your reinstatement fees waived.
