What information do I need before I ask the court for specialized driving privileges?
You will need the following information to complete your Specialized Driving Request.
- A certified copy of your OFFICIAL DRIVING RECORD. It can be purchased here: Official Driving Record. You can learn more about how to read your driving record here: Reading Driving Record
- Insurance Verification, which you can get here: Insurance Verification
- A copy of your driver’s license and your driver’s license number.
- A full list of the locations and addresses you would like to be able to drive to.
- A clear reason for needing specialized driving privileges.
How do I ask the court for specialized driving privileges?
- Click the “Download Instructions” button and read the instructions.
- After you’ve read the instructions, click the “I am ready to fill out the form” button and complete the forms.
- Take them to the courthouse or e-file them. You can e-file these forms, but print them first. Then, scan them back into your computer. E-filing fillable forms causes errors in the e-filing system. You can find out more about e-filing here: E-file Indiana
- You may have to file them in more than one county. The instructions tell you where you should file your forms.

How do I ask the court to waive my BMV reinstatement fees?
